*What is a HLA Report? HLA - Definition[ Human Leukocyte Antigen ] Tissue typing is a group of procedures that determines the type of histocompatibility antigens on a person's cells or tissues. This procedure is typically used prior to transplantation of tissues or organs. A procedure involving a test or a series of tests to determine the compatibility of tissues from a prospective donor and a recipient prior to transplantation. The immunological response of a recipient to a transplant from a donor is directed against many cell-surface histocompatibility antigens controlled by genes at many different loci. However, one of these loci, the major histocompatibility complex (MHC), controls antigens that evoke the strongest immunological response. The human MHC is known as the HLA system, which stands for the first (A) human leukocyte blood group system discovered. The success of transplantation is greatly dependent on the degree of histocompatibility (identity) between the donor and recipient, which is determined by the HLA complex. When the donor and recipient have a low degree of histocompatibility, the organ is said to be mismatched, and the recipient mounts an immune response against the donor antigen. By laboratory testing, the degree of antigenic similarity between the donor and the recipient and the degree of preexisting recipient sensitization to donor antigens (and therefore preformed antibodies) can be determined. This is known as *Cross-matching. If the patient has a known cardiac problem, or if the patient is above 40 years of age, an Exercise Tolerance Test Report and an Echocardiogram are also required. It is also recommended that we be informed of any pre-existing ailments, which the patient is suffering from, other than the Renal Failure. If so, investigations for that disease should be sent to us to evaluate the suitability of the candidate for the transplant. Patients who smoke tobacco, who drink alcohol on a daily basis, and those on regular substance use, will be advised accordingly in preparation for the surgery. List of Tests done on Donor:
An incision is made in the patient's lower abdomen, near the hip. The donor kidney is implanted near the bladder. Blood supply is restored to the donor kidney by connecting it to blood vessels supplying the patient's legs. Usually, patient's own kidneys are not removed. In some rare cases, one or both kidneys are removed during the transplant or at a later operation. Transplant typically takes about three hours. The donor kidney should begin to function immediately and patients typically remain in the hospital for apprx. 10 days if he/she is in his/her homeland. If a patient is away from home, he/she will be required to stay apprx. 21 days, to complete treatment to fly back home safely. The recipient can take a short walk a day or two later after the surgery. LIVER/HEART/LUNGS: TRANSPLANT SURGERY Surgery There are three [3] surgical procedures involved, which usually take about 6-8 hours. 1. Removal of the liver from the donor. If the liver is donated at a different location, it must be transferred to the transplant center under sterile refrigerated conditions within 8 to 20 hours. 2. Removal of the diseased liver from the patient. 3. Surgery to insert and connect the new liver. The surgery on the recipient is so detailed. The new liver is attached to the various blood vessels and bile ducts. When the surgery is completed, the patient goes to the recovery area. Upon discharge from the hospital the patient will be provided with a complete medical report in English, bearing the seal of the hospital and signature of the chief of the transplant team, thus enabling the physician in the homeland to continue with follow-up care upon the patient's return.
